Crynodeb
The post-holder will: Develop and maintain a database to actively manage and record patient level detail. Attend Cancer High Risk Meeting Prepare and attend the Multidisciplinary Team Meeting Collect and record data on behalf of AMS, which can be manipulated to allow the extraction of data for audit. Using data gathered identify, develop and implement, through collaborative working with Endoscopy, Colorectal, Upper GI and Gastro Teams improvements to the patient pathway and the Service. Collect and record patient information from Trust data recording systems on PPM and PAS Work closely with other specialities, managers, administration and Independent Sectors to deliver a coordinated, consistent management of patient suspected/cancer pathways. Ensure patients are booked into next appointments in line with national standards & Trust policies, communicating with key personnel to action upgrade to fast-track where required.
