We are seeking a dedicated and compassionate Social Worker to join our Adults Hospital Pathway Team at Grade 8 within our Facilities Services industry. In this vital role, you will be responsible for providing high-quality social work services and support to adults in hospital settings, ensuring their individual needs are met during their hospital stay and facilitating a smooth transition to the next stage of their care. Responsibilities
Provide emotional support and counseling to patients and their families during hospital stays.
Conduct thorough assessments of patients' social, emotional, and economic needs.
Develop and implement individualized discharge plans in collaboration with healthcare teams.
Facilitate communication between patients, families, and medical staff to ensure everyone is informed and supported.
Connect patients with community resources and services to assist them post-discharge.
Advocate for patients' rights and needs within the healthcare setting.
Maintain accurate and timely documentation of patient interactions, assessments, and care plans.
Requirements
Master's degree in Social Work from an accredited program.
Current licensure as a clinical social worker (LCSW or equivalent) relevant to the position.
Minimum of three years of experience in social work, preferably in a hospital or healthcare setting.
Strong knowledge of the healthcare system and community resources for adult patients.
Exceptional communication and interpersonal skills to effectively engage with diverse populations.
Ability to work collaboratively as part of a multidisciplinary team.
Experience in crisis intervention and strong problem-solving skills.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £21.35
