Responsibilities
Conduct assessments to identify the needs and strengths of adults with learning disabilities.
Develop, implement, and review personalized care plans.
Collaborate with healthcare professionals, families, and community organizations to coordinate services.
Provide advocacy and support to individuals to help them achieve their personal goals.
Monitor the progress of clients and adjust care plans as necessary.
Maintain accurate and up-to-date records and documentation in accordance with legal and organizational policies.
Participate in training and professional development opportunities to enhance skills and knowledge in the field.
Requirements
A recognized social work qualification (e.g., degree in social work, CQSW, CSS, or equivalent).
Registration with the relevant regulatory body (e.g., Social Work England).
Demonstrated experience working with adults with learning disabilities in a social work capacity.
Strong understanding of relevant legislation and best practices concerning learning disabilities.
Excellent communication and interpersonal skills to effectively interact with clients, families, and multidisciplinary teams.
Ability to assess situations critically and make informed decisions.
Commitment to professional development and continuous learning in the field of social work.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £22.95
