Description
We are seeking a dedicated Adults Social Worker to join our Locality team at Gr H. This role is vital to our mission of providing high-quality social care and support to adults in our community. As an Adults Social Worker, you will engage with individuals and families to assess their needs, develop tailored care plans, and ensure that services are successfully delivered. You will work collaboratively with external agencies, health professionals, and community organizations to promote the well-being and independence of clients. Responsibilities
Conduct assessments of clients' needs and circumstances.
Develop and implement personalized care plans that promote well-being and independence.
Provide ongoing support and guidance to clients and their families.
Collaborate with multidisciplinary teams and external agencies.
Advocate for clients' rights and access to appropriate services.
Monitor and review care plans regularly to ensure effectiveness and make adjustments as needed.
Document all interactions and interventions accurately and in a timely manner.
Requirements
A recognized social work qualification (e.g., Bachelor’s or Master’s in Social Work).
Registered with the relevant social work regulatory body.
Proven experience in adult social work, with a focus on locality services.
Strong understanding of social care legislation and policies.
Excellent communication and interpersonal skills.
Ability to work effectively within a team and independently when required.
Demonstrated problem-solving skills and a commitment to client advocacy.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £22.95
