Crynodeb
To support a process and toolkit for earlier recognition of children and young people who are at risk of significant distress leading to a breakdown in their home or care environment. To support define and implement a multiagency process for coming together around the child or young person to pool skills and develop a responsive care or support package. Work with partners across the health system eg Primary care, acute trusts. To support childrens social care to assess the full range of a young persons health needs and source appropriate care. To offer clinical advice to education, schools, colleges and employment settings when they are struggling to meet the needs of young people presenting in distress . To develop proposals for the innovative use of personal budgets . To understand the offer from VCSE organisations and how these might support individual children but also how this market might be developed into a formalised element of the systems response . To further Plymouths ambition to embed a trauma informed approach throughout system. To represent LSW CyPFS at system child focused panels supporting the health needs of young people, for example EHCP panels. Develop effective working relationships with key partners, young people and their families, Primary Care Networks, Education, Commissioners, Acute Trusts, Local Authorities, voluntary sector and staff side in order to deliver continuous improvement in the CYPF Service pathways. Continually review services and pathways, working with key stakeholders to effectively redesign pathways in order that they meet the needs of children and young people , and parents/carers. To manage and deliver high quality services which meet the needs of CYP and their parents and carers by working to ensure that performance standards are met and applied to a high standard consistently across the defined populations. Implementing effective processes for monitoring and managing activity, providing assurance via LSW governance structure for safe and effective service delivery.
