About the Role
We are seeking to recruit a Care Coordinator (Level 2) to join our Physical and Complex Care Division. You will deliver high-quality, person-centred support to adults with physical and complex needs, including palliative care and Dementia care. Working closely with health professionals, families, and colleagues, you will ensure each individual receives sensitive, dignified, and personalised support.

Key Responsibilities
Implement and review personalised care plans
Provide palliative care and Dementia care support
Complete required audits
Support rota cover
Assist with appointments, meals, domestic tasks, and personal care
Administer and record medication
Support with money management and household shopping
Promote community participation and meaningful activity
Facilitate education/employment opportunities
Provide emotional support
Apply active support and PBS (training provided)
Support with holidays and short breaks(Duties may be updated as service needs evolve.)
