Are you passionate about making a real difference in people’s lives? We’re looking for caring, flexible individuals to provide high-quality support to clients in their own homes, helping with daily living activities and promoting independence.

You’ll work shifts between 8am and 9pm, including weekends, and play a vital role in early identification of deteriorating patients, all while developing your skills through continuous professional development.

• To provide high quality care to clients under the indirect supervision of Team manager within the community bridging service.
• To support clients with activities of daily living in a clients home
• Ensure high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals
• To participate in continuous professional development and other team activities
• Carry out a range of duties within agreed competency skill set e.g. meal prep
• To ensure that all patients on the caseload are safe.

This vacancy is linked with the opportunity for Apprenticeship(for those eligible for Apprenticeships). This will support you to gain relevant knowledge, skills and experience that will help you achieve your potential in this role. You will not be required to undertake an apprenticeship if you have previously completed a relevant apprenticeship, NVQ or similar Healthcare qualification.
