JOB DESCRIPTION Job Title: Cancer Project Nurse for theTargeted Cancer Case Finding Programme Employer: Reporting to: PCN Clinical Director and PCN Manager Hours:2 Sessions per week Contract: Fixed Term 12 month Flexible working hours are possible. Experience:Previous Diabetes and Cancer experience. Location: Shrewsbury PCN across member Practices as required Works closely with Cancer Care Coordinator PCN Clinical Director Practice teams across Shrewsbury PCN Wider MDT asrequired Role Purpose TheCancer ProjectNurse will provide clinicalexpertiseand professional advice to support the delivery of the Shropshire,Telford and WrekinTargeted Cancer Case Finding Programme within Shrewsbury PCN. The post holder will work alongside the Cancer Care Coordinator, who leads on administration and patient coordination, and will provide clinical oversight, guidance, and decision-making support across the three targeted case finding pathways: Pancreatic cancer (new onset diabetes with weight loss) Prostate cancer (high-risk cohorts) Thrombocytosis-related cancer risk The role focuses on ensuring patients are clinically reviewed appropriately, investigations are interpreted correctly, and escalation/referral decisions are made safely and consistently, in line with the agreed specification. Key Responsibilities Clinical Leadership & Decision Support Provide senior clinical input and advice to support targeted cancer case finding activities across the PCN. Clinically review patient recordsidentifiedthrough searches todetermineappropriatenessoffurther investigation. Support decision-making around investigations, referrals, and safety-netting in line with agreed pathways. Provide clinical advice to the Cancer Care Coordinatorregardingpatient queries, symptoms, and escalation concerns. Liaise with the Clinical Director for complex cases or whereadditionalsenior clinical oversight isrequired. Targeted Case Finding Delivery Support delivery of the three targeted case findingprogrammes: Pancreatic cancer: review cases involving new onset diabetes and unintended weight loss; advise on further investigation and referral. Prostate cancer:supportPSA testing pathways, interpretation of results, and onward referral decisions. Thrombocytosis: support clinical assessment, investigation planning, and identification of potential underlying malignancy. Ensure investigations and referrals align with national and local guidance, including urgent suspected cancer pathways. Patient-Focused Clinical Input Where required, support clinical discussions with patients to explain investigations, results, and next steps. Ensureappropriate safety-nettingadvice is in place for patients not requiring immediate referral. Promote patient understanding and engagement throughout thecase findingprocess. Quality, Governance & Reporting Support Supportaccurateclinical documentation in patient records. Contribute to outcome reporting byvalidatingclinical elements of the datasubmittedto the ICB. Identifylearning, themes, or quality improvement opportunities arising from the programme. Ensure work is undertaken in line with PCN governance, information governance, and safeguarding requirements. Collaboration & Service Development Work collaboratively with practice teams, PCN staff, and the wider MDT. Provide feedback to improve pathways, processes, and patient experience. Contribute to the development of consistent, PCN-wide approaches to targeted casefinding.
