The post holder will work within the Recovery Hub and support the multi-disciplinary team. The post holderwill coordinate the safe andtimelyadmission anddischarge of customersto andfrom the Recovery Hub. Ensuringthat thepersonand theircarersare provided withahigh qualityexperiencefrom admission through to discharge. The post holder will contributeto the reduction in delayedtransfers of careand a reduced length of stay,supporting the wider Leeds health and social care systems. Responsibilities: To complete daily delegated tasks from the registered manager tofacilitatetimelyadmission toand dischargefrom the Recovery Hub. This will include but is not limited to: Coordinate withsystem partners and external organisationstoagreetimelyadmission of customers from both community and acute settings. To gather basic discharge information on admission and start discharge planning from the earliest opportunity Support the safe admission processes, specifically with responsibility for medicine reconciliation and transcription. Highlight as early aspossible customerswho have no reason toresidewithin the Recovery Hub and are ready for discharge, ensuring they are discharged in a safetimelymanner. Be an advocateof strengths-based social care andforpromotion of approaches to maximise independent livingand championing the city-wide Home First ambition. Coordinate referrals tosystem partners andexternal organisations such as Homecare Providers, Care Homes and Neighbourhood NHS teams and SkILS Reablement service. Contactingsystem partnerssuch as GPs, Community Services and Care Homes torequest/provideadditionalinformation and/or inform them ofdischarge planning. Attend andactionappropriate tasksas delegated by the RegisteredManager or Deputy ManagerfollowingMulti-DisciplinaryTeam meetings. To update theMulti-Disciplinary Teamon the progress of patient discharges and any delegated actions. Ensure No Reason to Reside data is reordered,maintained,updated. Escalate issues that cannot be resolved within agreedtimeframes. To attend family meetings and clinical meetings (e.g.MDT, Best Interest) and provide feedback post-meeting to the RegisteredManager or Deputy Manager andappropriate personsinvolved in thecustomerscareas necessary. Liaise with the teamon a daily basisfor an updated position oncustomer progress. To communicate inan appropriateand effective manner tocustomers,familiesor carers where there may be barriers to communication such as language barriers, hearingimpairmentsor cognition issues. To updatecustomerrecords asappropriate,followingthe Councilspolicies and procedures. Maintain personal competencyin the use of electronic care records such as CIS and PPM+ Tocomply withthe requirements of all Leeds City Council policies,proceduresand staff instructions, including responsibilities under the Health and Safety Policy and Procedures. To actively promote and support Leeds City Councils policies, best practice and values on diversity and equality of opportunity. Undertake a range of duties as delegated by the Registered Manager or Deputy Manager, working on own initiative within agreed policies and procedures. This includes but is not limited to thefollowing:- Supporting customers with daily living tasks including personal care. To ensure all customersidentifiedto be discharged each day are ready at the agreed time. This will involve completing a Discharge checklist. To orientate and welcome newcustomer, families and carers to theRecovery Hubandprovide them with relevant service information. Compilingcustomerbelongings and checking all are present against the patient property form. Confirming discharge with relatives, care homes,carersor community hospitals/ other NHS organisations asappropriate. Confirming transport arrangements with relatives and transport services, booking hospital transport ifrequired(in the absence of theadmin team). Ensuring thecustomerhas their medications and discharge summary. Confirming post discharge arrangements (e.g. outpatient appointments). Qualifications:Commitment to undertake a Level 3 Adult Care Worker Apprenticeship within 12 months of commencement of the post. Essential Requirements: It is essential that the candidate should be able todemonstratethe following criteria for the post. Candidates will only be shortlisted for interview if they candemonstratethat they meet all the essential requirements. Skills Ability to undertake moving and handling Ability to efficiently prepare, communicate and present management information to a range of partners and stakeholders. Ability to work unsupervised and on aone to onebasis with customers anddemonstratean ability to work on own initiativeon a daily basis. Ability to make decisions and work flexibly as part of a team. Ability to work positively and contribute to multi agency working arrangements Ability to complete person centred risk assessments and customer support plans Ability to review risk assessment and support plans in a responsive andperson centredway Ability toassistthe manager with medication administration and management within RH17 medication policy and procedure. Ability toparticipatein formal reviews including the provider-led annual reviews andparticipatein multi-agency meetings. Ability tomaintainpositive relationships with partners Ability todemonstrateeffective support management and to be able to deputise in the absence of the Manager and Deputy Manager for dutiescommensuratewith the grade of this post. Ability toseek, interpret and evaluate information from a variety of sources. Ability to make decisions using all available sources of information, having regard for the consequences and todemonstratethe use of autonomy in reaching such decisions. Ability to deal with any sensitive issues Ability to undertake formal staff supervision. Ability to work with and develop principles of social inclusion and recovery principles. Ability to work with a variety of electronic communications, datasystemsand technology. Knowledge Demonstrate knowledge of current, trends, policies,legislationand any regulatory body requirements, in relation to vulnerable adults within a social care setting. Knowledge of the Care Quality Commissions regulatory functions. Demonstrate an understanding of the importance of confidentiality, dataprotectionand information governance requirements Demonstrate knowledge of the needs of older people, theconceptsand methods of delivering a recovery model and on understanding the needs of customers accessing the service. Demonstrate knowledge of Quality Assurance systems and of methods of securing service improvement. Demonstrate an awareness of the needs associated with vulnerable adults, with complex mental health and social needs. Demonstrate an understanding of equality and diversity issues within Leeds and the local community. Requirement to complete the Care Certificate within 12 weeks of commencement of the post. Demonstrate functional skills in literacy and numeracy at Level 2 or equivalent. Commitment to undertake a Level 3 Adult Care Worker Apprenticeship within 12 months of commencement of the post. Demonstrate knowledge how to foster andmaintaineffective partnerships Demonstrate an understanding of the Local Authority, its function and organisational structures and values. Demonstrate an ability toidentifyproblems and seekappropriate solutions Demonstrate knowledge of strength based social work and the social model of disability. Experience: Experience in a support management role in a health, or social care / community setting Of advocacy and / or user involvement. Of using own initiative and ofdemonstratinga commitment to service development. Of contributing to staff appraisal, supervision, staff development within a social care or health care setting.
