Crynodeb
Build trusting and collaborative relationships with a range of service users and tailoring the service to meet their needs. Work closely with carers, families, children and other services involved with service users. Work across all Bristol inpatient services to meet the needs of an efficient Discharge Facilitation Service. Work across all inpatient PICU and Acute and Older Adult service and to liaise where appropriate with The Priory Hospital, Bristol. Attend inpatient wards to work with service users, clinicians, ward staff and the bed manager to facilitate the discharge of service users.
