As an IDS Senior Coordinator, you'll be on the frontline of hospital discharge - the vital link between patients, families, ward teams, and community services. This is a dynamic, ward-based role where you'll drive discharge momentum, gather the information needed for safe discharges, and help patients get home when they're medically fit.

Working autonomously with clinical awareness, you'll be trusted to identify barriers, coordinate actions, and know when to escalate. Every day you'll see the direct impact of your work - patients going home, beds freed for those who need them, and flow improving across the hospital.

Please note:



You'll be embedded on hospital wards as the discharge coordination expert. Within 48 hours of every admission, you'll have gathered essential collateral from the patient, clinical systems, care providers, and families. You'll attend daily board rounds, actively discussing and appropriately challenging delays, ensuring discharge actions have clear owners and timeframes.

Working closely with your Case Manager, you'll coordinate assessments, initiate referrals, and update patients and families about progress. You'll document everything in CareFlow using SBAR format, maintain real-time Flow Board accuracy, and escalate concerns promptly when they're outside your scope.

This role requires strong IT skills, excellent communication across multiple stakeholders, and the ability to manage competing priorities while maintaining attention to detail. You'll need knowledge of the Mental Capacity Act, safeguarding procedures, and discharge pathways, with an understanding of what makes a safe, timely discharge.

· Attend daily ward board rounds as IDS representative, challenging delays and driving discharge momentum

· Gather essential collateral within 48 hours using IDS triggers - liaise with GPs, care providers, families, and community services

· Hold early discharge conversations with patients and families, setting realistic expectations from admission

· Undertake baseline assessments with the MDT, ensuring admission and social assessments completed within 24 hours

· Document findings accurately in CareFlow using SBAR format and maintain real-time Flow Board updates

· Coordinate discharge actions with clear timeframes and action owners across ward teams and community partners

· Recognise when specialist referrals are needed and initiate appropriately

· Work autonomously within scope of practice, escalating to Case Manager or Operational Leads when required

· Meet daily with Case Manager to review patient progress and coordinate complex cases

· Support ward education on discharge processes and Managing Expectations protocol.
