Crynodeb
You will support children, young people and their families in the community who may be experiencing a mental health crisis or at risk of an inpatient admission. You will also offer support to young people who are transitioning from an inpatient admission back into the community. You will play a pivotal part in informing their care and work closely alongside the core CAMHS team and external agencies and professionals to ensure that the best possible care and outcomes are achieved for the young people you support. This role will also provide meal supports for children and young people with an eating disorder, under the direction of the Eating Disorder team and Getting More Help Keyworker.
