ABOUT THE SERVICE
The Langford Centre is a purpose built, state-of-the-art facility providing exceptional care for individuals with complex mental health needs, including dementia and neurodegenerative disorders.
Across our 6 wards we provides low secure, high dependency inpatient rehabilitation and acute services; Our aim is to support our patients from the point of admission to engage in their recovery pathway to promote successful reintegration back in to the community.
ABOUT THE ROLE
We have an opportunity for a Social Worker to join our team. Working alongside a multi-disciplinary team, you will have clinical responsibility for in-patients whilst providing support to patients and their families. This is achieved through a blend of treatment and rehabilitation during their stay at the Langford Centre. The role will also have responsibility for safeguarding issues, training and function within the Care Programme Approach.
MAIN DUTIES OF THE SOCIAL WORKER:
o Assessment of patient needs for social work interventions throughout admission including 1-1 and group work with patients.
o Ensuring that patients rights are protected under the Mental Health Act 1983, Human Rights Act, Safeguarding of Vulnerable Adults (SOVA), and Community Care legislation.
o Advocating for patients or ensuring the involvement of appropriate advocacy services.
o Preparation and presentation of Mental Health Tribunal and social history reports.
o Working with carers and family members including visits to family homes, liaison with health and social care services, other voluntary or statutory agencies, and identifying and assessing appropriate community resources.
o Planning for Discharge and recording Section 117 Aftercare arrangements.
o Attendance at CPA meetings and presentation of evidence of social work intervention.
o Completing Clinical audits and prepare reports.
o Responsibility for carrying out capacity Assessments.
o Prepare monthly reports for Corporate Clinical Governance meeting.
o Responsibility for safeguarding issues, including main point of contact for advice, referrals for safeguarding.
o Undertake home visits and complete relevant history / risk assessment etc. as required for the safe management of patients with forensic history.
o Identify, visit and assess resources that may be considered appropriate for service users moving into the community and or other residential settings.
o Maintain up-to-date records in clinical notes of all service user contacts.
o Observe patient confidentiality other than any risks that may be agreed by the clinical team.
THE SUCCESSFUL SOCIAL WORKER APPLICANT SHOULD HAVE:
o Verifiable experience of working as a Social Worker in a similar setting.
o Experience of working with individuals with mental health issues and / or person with forensic history and their family / carers in hospital and /or community settings.
o Experience of working with people with challenging behaviors.
o The willingness and ability to engage in further education and training relevant to social worker practice, and secure conditions.
o The willingness to develop a working understanding and knowledge of managing patients with a forensic history.
