Description
We are seeking a dedicated and compassionate Social Worker - Transition (Gr H) to join our dynamic team. The ideal candidate will play a crucial role in supporting individuals as they navigate significant life transitions, helping them to overcome challenges and achieve their personal goals. This position involves working closely with clients, families, and various community resources to develop tailored support plans that address their unique needs. As a Social Worker in this role, you will assess clients' situations, provide counseling, and advocate for necessary services to facilitate smooth transitions, whether it be returning home from an institution, adjusting to new living arrangements, or entering community programs. The successful applicant will possess strong interpersonal skills, a deep understanding of social work practices, and a genuine commitment to making a positive difference in the lives of those we serve. In addition to direct client interactions, you will collaborate with multidisciplinary teams to ensure comprehensive support strategies are in place. Responsibilities
Conduct assessments to determine the needs of clients undergoing transition.
Develop and implement individualized service plans in collaboration with clients and their families.
Provide counseling and support to clients during times of change or crisis.
Act as an advocate for clients, helping them access necessary resources and services.
Facilitate communication between clients, families, and other service providers.
Monitor client progress and adapt service plans as needed to ensure continued support and success.
Maintain accurate and timely documentation of client interactions and progress.
Requirements
Master's degree in Social Work (MSW) from an accredited program.
Current state licensure as a social worker.
At least 2 years of experience working in social work, preferably in transition services.
Strong knowledge of community resources and support services available for clients.
Excellent communication and interpersonal skills, with the ability to build rapport with clients.
Proficient in case management practices and documentation standards.
Ability to work collaboratively as part of a multidisciplinary team.
Hours Per Week:
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £23.46
