Crynodeb
The post-holder will be required to work within the multi-disciplinary team structure of the Community Services, providing specialist professional advice and expertise in learning disabilities. The job-holder will be required to carry a caseload of service users within the Community Service. Where appropriate this includes roles of care co-ordinator, placement monitor, care manager and social supervisor. Although based in Harrow the post includes the requirement to manage cases outside the Harrow area. In addition to having a caseload, the post-holder will be required to adopt an effective and positive team approach and work with the other Care Co-ordinators to ensure effective team delivery of client/ carers care. It is expected that the post-holder will make a valuable contribution to multi-disciplinary team assessments, the formulation and implementation of MDT care plans, while working closely with individuals and teams in enabling recovery and independence. He/she will establish and maintain a high standard of care, ensuring that the quality of care provided is continually informed by the best evidence-based practice. Please see the detailed JD attached.
