Crynodeb
To implement the National Framework for Continuing Healthcare (CHC) and Funded Nursing Care (FNC) in a timely, efficient and transparent manner and in conjunction with the individual /family and Multidisciplinary Team. To provide high quality holistic assessments and reassessments to enable the application of the National Framework and to identify the level of care needs and to make recommendations for the commissioning of care. To support the joint working with all organisations in relation to the relevant hospital discharge pathways under the discharge to assess model (4 pathways).
