Summary
Diabetes Practice Nurse – Gloucester
A forward-thinking, state-of-the-art training practice in Gloucester is seeking an experienced and enthusiastic Diabetes Practice Nurse to join its progressive and supportive team.
This modern practice is widely recognised for its innovative approach to primary care and its strong reputation as a GP training environment. The successful candidate will have the opportunity to take a lead role in the delivery of diabetes care, supporting patients with both newly diagnosed and long-term conditions. They will work closely with GPs, practice nurses, HCAs, pharmacists, and other allied health professionals to ensure seamless and holistic care.
Key responsibilities will include:

Providing evidence-based diabetes management, including reviews, medication titration, and patient education

Running nurse-led diabetes clinics to empower patients in self-management of their condition

Supporting wider chronic disease management where appropriate

Actively contributing to the development of clinical protocols and pathways

Participating in quality improvement initiatives, audits, and health promotion campaigns

Supporting the practice’s role as a teaching and training environment for medical, nursing, and allied health learners
The ideal candidate will:

Be a qualified Practice Nurse with significant experience in diabetes care and chronic disease management

Hold relevant training and qualifications in diabetes management and insulin initiation (or be willing to work towards them)

Demonstrate excellent clinical, organisational, and interpersonal skills

Be motivated to contribute ideas, embrace innovation, and support the practice’s vision of continuous improvement

Have a patient-centred approach with a strong focus on education, empowerment, and long-term outcomes
