Your role and responsibilities
You will be
- carrying out Care Act Assessments and Mental Capacity Assessments.
- assessing functional ability and promoting digital care technology.
- providing advice and guidance to customers, families, and health professionals.
- collaborating with NHS colleagues and social care teams to deliver joined-up support.
- supporting community referrals to prevent hospital admissions.
- managing referrals, including benefits tracking and eligibility decisions for care and support needs.
You will also have the opportunity to work flexibly, including working from home.
You will need
We hope you’ll bring:
- A strong understanding of adult social care and the Care Act and other Frameworks.
- Experience working within a health or social care setting.
- Excellent communication and decision-making skills.
- A proactive, person-centred approach to supporting independence.

The team
We currently cover Mildenhall and Newmarket and have two separate Integrated Teams and work across Mildenhall, Brandon and Newmarket with our health colleagues. You will be expected to work from Newmarket sometimes. Due to the team operating across two bases and the need to engage directly with the community, travelling will be required as part of this role.
