Medway Community Healthcare is excited to launch the “This is Me” Project, a dynamic initiative aimed at amplifying the voices of children and young people across Medway. We are seeking a motivated and compassionate Project Lead to drive this work forward over the next 12 months.
As Project Lead, you will:
• Coordinate and deliver the “This is Me” project across Medway.
• Work collaboratively with children, families, schools, and community partners.
• Facilitate workshops, creative sessions, and storytelling activities.
• Capture and showcase the lived experiences of children in meaningful ways.
• Monitor progress, evaluate impact, and report on outcomes.

• Experience working with children and young people in health, education, or community settings.
• Strong project management and communication skills.
• A creative and inclusive approach to engagement.
• Ability to work independently and as part of a multidisciplinary team.

Qualifications
You will need to hold a relevant degree or equivalent experience gained within a health and social care setting (e.g. nursing, social work, occupational therapy, psychology, education, Child development).
You will be able to evidence Specialist qualifications relevant to area of practice
Experience
Experience of working with children with additional needs.
You’ll need to have experience of working with children and young people requiring complex and skilled interventions. As well as experience of and working with children comorbid difficulties and special needs.
You will be experienced in supporting others (mentor / educator / supervisor) experience
You will have experience of delivering training to others
You will have practical experience in managing or assisting with health related projects/complex caseloads
Experience of delivering groups/support classes
Experience with the use of Microsoft Office and a variety of IT packages
Special knowledge / expertise
Understanding of social communication and behavioural needs of children.
You will have a good knowledge and understanding of local and national frameworks relevant to social models of disability/SEND and its application across relevant systems such as health care / social care and educational structures
Experience working with children with social communication and behavioural needs.

Well developed communication skills

Skills and abilities
You’ll need have sound communication skills in order to understand and communicate both verbally and in writing some of which may be complex and or sensitive.
You will need to have a customer focussed approach which is essential to;
· Provide children and young people and their families with the best possible experience
· represent the Trust when talking to and working with patients, colleagues and visitors.
· Work together with colleagues and other teams across the Trust.
You will need to be able to be able to use IT systems and databases.
You will need to be able to work well under pressure and be able to organise your own work.
You’ll need to be adaptable in work as things change as part of working in a wider team.
You will have the skill to focus on the details with the ability to spot problems.
You will know when you are able to correct or when to escalate
Car driver
Proficient in use of computer packages such as word, excel, publisher

You will be able to support the implementation of local and national agendas under the guidance of the senior Practitioner and manager Band 4 staff and below.
