Medway Community Healthcare is proud to launch the “This is Me” Project, a vibrant initiative designed to amplify the voices of children and young people across Medway. We are looking for a dedicated Project Support Practitionerto help bring this vision to life over the next 12 months. Apply now and help us ensure every child can proudly say, “This is Me.”

As Project Support Practitioner, you will:
• Assist in the planning and delivery of creative sessions and workshops.
• Support children and young people to express themselves through storytelling, art, and other media.
• Help coordinate project activities, resources, and logistics.
• Build positive relationships with families, schools, and community partners.
• Contribute to monitoring and evaluation of project outcomes.

Qualifications

GCSE or equivalent in English.experience gained within a health and social care setting (e.g. nursing, social work, occupational therapy, education, Child development).



Diploma in health and social care

Experience

Experience of working with children with additional needs.



You will be experienced in supporting others (mentor / educator / supervisor) experience

You will have experience of delivering training to others



You will have practical experience in managing or assisting with health related projects/complex caseloads





Experience of delivering groups/support classes



Experience with the use of Microsoft Office and a variety of IT packages

Special knowledge / expertise

Understanding of social communication and behavioural needs of children.



You will have a good knowledge and understanding of local and national frameworks relevant to social models of disability/SEND and its application across relevant systems such as health care / social care and educational structures

Experience working with children with social communication and behavioural needs.

Disposition, adjustment, attitude and commitment

Well developed communication skills

Ability to work as part of a team as well as independently

You will need to have a customer focussed approach which is essential to;

· Provide children and young people and their families with the best possible experience
