We are looking for a new member of the dementia crisis team, providing care and therapeutic intervention to patients in a range of community settings under the guidance of professionally qualified members of the team.

The post holder should have a passion for assisting disabled and/or vulnerable adults to live at home with dignity and independence while supporting them by providing rehabilitative care.

The post holder would need to undertake a range of delegated clinical patient care duties in a range of settings, to maintain patient records. to assist in the ongoing development and evaluation of the service

Qualifications

NVQ Level 2 in healthcare or equivalent



Qualification relating to caring for people living with dementia, or NVQ level 3

Experience

Proven experience working with vulnerable adults with complex and varying needs within a social or health care setting



Multi-disciplinary teamwork



Understanding of the need for enabling patients to achieve optimum independence



Experience of working with people with dementia



Experience of working in a community setting

Special knowledge / expertise

Demonstrate a good working knowledge of dementia and importance of person centred care/therapy



Awareness of support available for older people

Disposition, adjustment, attitude and commitment

Good interpersonal skills



Flexibility and adaptability to change.







Practical / intellectual skills

Fundamental IT Skills



Ability to work on own initiative and unsupervised



Ability to carry out rehabilitation/therapeutic activities
