We are delighted to be able to offer an opportunity for a highly motivated, ambitious individual to join the Newcastle Hospitals Palliative Care Service. This is a trust wide liaison service.
This post will be based within the Community Specialist Care Home Support Team, working closely with one of our Palliative Medicine Consultants to provide support and advance care planning to patients in the care home setting. This post attracts 4 PAs for in hours NUTH work.
The successful candidates should have excellent communication skills and experience in the management and care of palliative patients with cancer and non-cancer diagnoses, ideally in a community setting. They should be a good team player, and have skills to work well within in a multi-disciplinary team of experienced nurse specialists, as well as in a liaison role with General Practitioners.
The successful candidates will join this liaison service, consisting of doctors, nurse specialists, health care assistants and psychologists, which supports the delivery of high quality palliative and end of life care to patients within Newcastle Hospitals and those living in Newcastle community.
The successful applicant will work in the Community within the Newcastle Hospitals Specialist Care Home Support Team. They will work closely with one of our Palliative Medicine Consultants and the nurse specialists in the team to support the recognition of patients identified as being in the last year of life and offer opportunities for Advance Care Planning, aiming to improve the quality of life and death for care home residents in Newcastle.

· Support the nurses within the specialist care home support team to identify residents likely to be in their last year of life and offer proactive care planning through MDT meetings, advice and joint visiting
· Liaise with hospital specialists where needed to establish appropriate treatment plans for care home residents
· Work closely / liaise with General Practitioners to share care planning
· Participate in and/or lead reflective practice sessions with the team and care home staff
