Description
We are seeking a dedicated and experienced individual for the position of ASW - Senior Practitioner - Independence & Access. This role plays a pivotal part in supporting our mission to empower individuals to achieve independence and access the services they need. You will work collaboratively with a multidisciplinary team to assess, plan, and implement tailored support strategies for clients, taking into consideration their individual needs and aspirations. As a Senior Practitioner, you will also be responsible for mentoring and coaching other team members, fostering professional development and ensuring high standards of practice within the team. Your expertise will be crucial in navigating complex situations and providing innovative solutions that enhance service delivery. We are looking for someone who is not only knowledgeable about current legislation and best practices in social care but also possesses the interpersonal skills necessary to engage effectively with clients, families, and stakeholders. Responsibilities
Lead assessments of clients' needs and create personalized support plans to promote independence.
Work collaboratively with internal and external stakeholders to ensure effective service delivery.
Mentor and provide supervision to junior staff members to enhance their professional growth.
Conduct regular reviews of client progress and adjust support plans as necessary to align with their evolving needs.
Advocate for clients' rights and access to services, ensuring their voices are heard.
Monitor and evaluate the effectiveness of interventions and services provided to clients.
Stay updated on relevant legislation, policies, and best practices in the field of social care.
Requirements
A recognized qualification in social work or a related field, with appropriate registrations.
Minimum of 5 years experience in social care or a related environment, ideally in a senior capacity.
In-depth knowledge of policies, legislation, and best practices relevant to independence and access services.
Strong assessment and care planning skills, with a focus on person-centered approaches.
Excellent communication and interpersonal skills for effective collaboration with clients and team members.
Proven ability to manage complex cases and provide innovative solutions to challenges.
Demonstrated commitment to continuous professional development and a passion for empowering individuals.

End Time: 17:00
Pay Per Hour: £27.92
