Responsibilities
Conduct comprehensive assessments of clients' needs and circumstances.
Develop, implement, and review individualized care plans in collaboration with clients and families.
Coordinate with various service providers to ensure the delivery of services aligned with care plans.
Monitor and evaluate the effectiveness of care and support services, making adjustments as necessary.
Act as an advocate for clients, ensuring their rights and preferences are respected and upheld.
Maintain accurate and up-to-date case records and documentation in compliance with organizational policies.
Participate in multi-disciplinary team meetings to share insights and develop collective strategies for client support.
Requirements
A relevant undergraduate degree in social work, psychology, or a related field.
Experience working in adult social care or a similar healthcare setting is highly desirable.
Understanding of adult safeguarding procedures and legislation.
Strong communication and interpersonal skills to effectively engage with clients and colleagues.
Ability to work independently as well as collaboratively within a team environment.
Demonstrated organizational skills and the ability to manage a caseload effectively.
Willingness to undertake further training and development to enhance professional skills.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £23.86
