Job Title: Lead Nurse Location: Queen Square Medical Practice Reporting to: Clinically: GP Partners Managerially: Practice Director Band & Salary: Equivalent to AfC Band7 (Based on experience) Hours: Full-time, flexible working (part-time considered) Job Purpose Provide clinical leadership and management for the nursing team, ensuring safe, effective, evidence-based care across long-term conditions LTCs, acute care, immunisations, and health promotions. Support practice and PCN targets e.g. QOF, enhanced services, infection control. Act as a senior member of the practice management team, contributing to clinical governance and service development. Key Responsibilities1. Leadership & Team Management Lead, mentor, appraise, and develop the nursing team; coordinate rotas and workloads. Chair nursing and governance meetings; represent nursing at practice/PCN forums. Foster a supportive, learning-oriented culture and supervise clinical development of students, new nurses. 2. Clinical Leadership & Service Delivery Deliver and oversee LTC management e.g. diabetes, asthma, CVD, including audits and QOF performance. Lead immunisation and vaccination programmes for adults/children and travel clinics. Provide autonomous clinical care including acute presentations, wound care, contraception, cytology and prescribe within PGDs or as qualified. Act as the practices Infection Prevention & Control lead. 3. Clinical Governance & Quality Improvement Interpret and implement national guidelines NICE, NSF, audit clinical practice, and introduce improvements. Monitor and achieve performance metrics QOF, LCS, PCN targets. Ensure safe vaccine/drug storage, minimise risk through robust infection control, and oversee vaccination fridges. 4. Administration & Training Maintain accurate, coded clinical records Read/SNOMED, protecting patient confidentiality. Manage nurse rotas, cover, study leave; support appraisal and revalidation processes. Provide training and supervision to nurses, HCAs, students; support professional development. 5. Service Development & Collaboration Identify opportunities to extend nursing services e.g. respiratory diagnostic clinics, LD clinics. Collaborate with PCN staff pharmacists, paramedics, social prescribers and external agencies. Actively participate in practice/PCN management decisions and strategy planning. Experience Essential Substantial experience in primary care nursing chronic disease management, immunisations, etc. Experience implementing clinical guidelines, delivering QOF/LCS targets, and conducting audits. Proven experience leading or managing nursing or HCA staff, including appraisals. Experience with infection control and IPC lead role. Experience Desirable Data entry and working with practice management systems. Coordinating a nursing or practice department; cross-site or PCN working. Skills & Knowledge Essential Clinical skills: wound care, immunisations, cytology, ECGs, spirometry, venepuncture, minor procedures. Strong leadership, communication verbal & written, and mentoring skills. Knowledge of primary care contracting QOF, LCS, PCN, clinical governance, NICE/NSF guidelines. Data analysis capability, audit experience, and ability to implement changes effectively. Personal Attributes Motivated, flexible, and responsive to change; solution-focused.
