Job responsibilities You will: Working within a team of Continuing Healthcare Practitioners, ensuring a high-quality service is delivered Supporting the Continuing Healthcare team to ensure that workstream plans are fulfilled and supervised effectively to ensure successful delivery Be responsible for managing own workload as delegated by the line manager Escalate any difficulties with completing the delegated tasks to appropriate line manager Support the Continuing Healthcare team to ensure National Framework compliant processes and local processes are completed in a timely manner Maintain accurate records on teams database system Attend team meeting including team supervision sessions for clinical support, identifying and sharing good practice Actively supporting the Continuing Healthcare team in working towards goals and objectives Communicating information, risks, issues and dependencies, including briefings and reports Maintaining up-to-date knowledge of legislation, policies and guidance Maintaining constructive relationships with a broad range of internal and external stakeholders Working closely with care providers involved in delivery of the service Delivering against objectives, achieving quality outcomes, prioritising own workload and working to tight deadlines Ensuring own compliance with all statutory and mandatory training Fostering transparency and a person-centred approach with patients, families and representatives Carrying out other appropriate delegated duties as required and appropriate clinical oversight Promotion of the use of Personal Health Budgets for eligible individuals as a means of their having control over their care and achieving best possible outcomes whilst being cost effective and value for money Ensure the principles and duties of safeguarding are holistically, consistently and contentiously applied Contribute to maintenance and monitoring of service standards around quality Comply with NHS Codes of Conduct and Standard Business Conduct and/or NHS Trust Standards of Business Conduct policies The post holder will be responsible for ensuring their mandatory training is up to date The job description and person specification are an outline of the task, responsibilities and outcomes of the role.
