Nursing associates are a new role within the nursing team in England, designed to bridge the gap between healthcare assistants and registered nurses. You will have established expereince of providing hands-on, person-centered care in various health care settings, working with patients of all ages. You will be registered and regulated by the Nursing and Midwifery Council (NMC) and have your own distinct standards of proficiency .

This position offers the opportunity to work flexibly and remotely in supporting Personalised Healthcare Commissioning services to deliver Continuing healthcare.

You will play a key role in supporting the Continuing Healthcare service across the region. Working within the remit of the National Framework for Continuing Healthcare, the post holder will deliver an effective and competent level of service and consistently deliver a client-focused service.

You will assist and support in the facilitation of multi-disciplinary assessments and patient reviews, in line with standard operating policy and procedures, and ensuring The National Framework for NHS Continuing Healthcare and Funded-Nursing Care is implemented and complied with, according to the principles and processes.

As an Associate Practitioner, you will be expected to have completed a Level 5 Continuing Healthcare Associate Practitioner Apprenticeship, working within a dynamic team, delivering an effective service as part of the NHS Continuing Healthcare team. You must have basic knowledge and experience of working within the health sector and being appropriately assessed against the NHS England CHC Competency Framework. You will have IT and record keeping skills. As an experienced health practitioner you will be able to work independently and as part of the team.

The post holder should possess basic knowledge of the NHS Framework for Continuing Healthcare and NHS Funded Nursing Care; along with local policy and practice related to CHC.

Job responsibilities



You will:

· Working within a team of Continuing Healthcare Practitioners, ensuring a high-quality service is delivered



· Supporting the Continuing Healthcare team to ensure that workstream plans are fulfilled and supervised effectively to ensure successful delivery



· Be responsible for managing own workload as delegated by the line manager



· Escalate any difficulties with completing the delegated tasks to appropriate line manager



· Support the Continuing Healthcare team to ensure National Framework compliant processes and local processes are completed in a timely manner



· Maintain accurate records on team’s database system



· Attend team meeting including team supervision sessions for clinical support, identifying and sharing good practice



· Actively supporting the Continuing Healthcare team in working towards goals and objectives



· Communicating information, risks, issues and dependencies, including briefings and reports



· Maintaining up-to-date knowledge of legislation, policies and guidance



· Maintaining constructive relationships with a broad range of internal and external stakeholders



· Working closely with care providers involved in delivery of the service



· Delivering against objectives, achieving quality outcomes, prioritising own workload and working to tight deadlines



· Ensuring own compliance with all statutory and mandatory training



· Fostering transparency and a person-centred approach with patients, families and representatives



· Carrying out other appropriate delegated duties as required and appropriate clinical oversight



· Promotion of the use of Personal Health Budgets for eligible individuals as a means of their having control over their care and achieving best possible outcomes whilst being cost effective and value for money



· Ensure the principles and duties of safeguarding are holistically, consistently and contentiously applied



· Contribute to maintenance and monitoring of service standards around quality



· Comply with NHS Codes of Conduct and Standard Business Conduct and/or NHS Trust Standards of Business Conduct policies



· The post holder will be responsible for ensuring their mandatory training is up to date

Training And Development:

· Attend all mandatory training relevant for post

· Identify own training needs, in consultation with line manager, and attend relevant courses / study days to enhance personal development

· Attend and actively participate in an Annual Personal Development Review with line manager

· Actively seek out learning opportunities via networking with peers in other organisations and steering group activities





Organisational Responsibilities:

· The post holder is required to follow all MLCSU policies and procedures relevant for role

· Participate in and contribute to relevant professional meetings as required

· Maintain absolute confidentiality of information
