Crynodeb
It is an exciting time to come join us in Adult Integrated Care. Using a strengths based approach to care, our vision is to promote independence and meeting the health and wellbeing needs of vulnerable people. We are looking to recruit enthusiastic and hardworking, Care Coordinator within our Community team for learning disabilities West, in the Wakefield District. You will work closely with our key partners in the CTLD Health and Intensive Support Team, to carry out person led assessments with people aged 18 and over with a learning disability. Based on strengths to promote positive change, self-determination and to prevent harm to people with an appearance of social care needs, in order for people to achieve their personal outcomes.
The team consists of social workers, care co-ordinators and co-ordination support officers. As a care coordinator in the team, you will be passionate about making a difference, and ready to embrace change. Working Monday to Friday 8:30am – 5pm and to 4.30pm on a Friday.
As a team player, you will need to demonstrate an ability to, work under pressure, be well organised and able to effectively prioritise and risk assess/manage a caseload. You will have or be ready to learn to build your skills and experience, knowledge and have a commitment to strength-based assessments and support planning. The ability to travel is essential.
