Description
We are seeking a qualified ASW Social Worker with a minimum of 2 years post-qualification experience (PQE) in adult social work to join our dedicated team. Responsibilities
Conduct comprehensive assessments of clients' needs and circumstances.
Develop and implement individualized care plans in collaboration with clients.
Facilitate access to services and resources for clients, including healthcare and community support.
Monitor and review the progress of clients in relation to their care plans.
Provide advocacy and support to clients to ensure their voices are heard.
Collaborate with other professionals and stakeholders to coordinate care and support.
Maintain accurate and up-to-date records in compliance with data protection legislation.
Requirements
Minimum of 2 years post-qualification experience (PQE) in adult social work.
Hold a relevant social work qualification (e.g., Degree in Social Work, MA in Social Work).
Hold a valid Social Work registration with the appropriate regulatory body.
Demonstrated understanding of relevant legislation and frameworks affecting adult social care.
Proficient in conducting assessments and developing client-centered care plans.
Excellent communication and interpersonal skills to effectively engage with clients and stakeholders.
Strong organizational and time management skills to handle a diverse caseload.
Hours Per Week: 37.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £22.76
