Support the NETT team facilitating smooth transitions for complex neurological patients from hospital/rehab units back into the community. Your short term, intensive support (up to 12 weeks) will directly contribute to reducing hospital stays and preventing readmissions
Within CNRT, you'll work with individuals over 18 with a range of neurological conditions, providing vital support for recovery, managing difficulties, and promoting wellbeing. Your understanding of progressive conditions and the importance of ongoing neuro disability management will be key
Manage your case work with a degree of autonomy, utilising your professional judgment while benefiting from the support and guidance of your Deputy Team Manager
Essential skills in delivering adult social care interventions, a strong understanding of the principles of neurorehabilitation, ability to work effectively in a fast paced environment with a focus on short-term interventions and achieving outcomes
Excellent communication and interpersonal skills to collaborate effectively within multidisciplinary teams and ability to manage your own workload and work with a degree of autonomy
