We are seeking a experienced Social Worker to play a vital role within our Community Neuro Rehabilitation Team (CNRT) and the Neuro-rehab Enhanced Transition Team (NETT). This is an exciting opportunity to work with multidisciplinary teams, alongside Occupational Therapists, Physiotherapists, Speech and Language Therapists, Neuropsychologists, Nurses, Doctors, and rehab support workers.

Key Responsibilities:
- Collaborative working, contributing your social care expertise to holistic patient plans and attending weekly in person MDM meetings for both NETT (Thursdays 10-11 AM at Pulross B, Lambeth Community Care Centre, London, SE11 4TX) and CNRT (Thursdays 1-2:30 PM at Bermondsey health centre 108 Grange Road, London SE1 3BW ) in addition to online huddles on Fridays 8 to 9 AM.
- Provide crucial adult social care interventions, including Care Act Assessments, MCA, Safeguarding, advice, information, and referrals, all with a focus on achieving both individual patient goals and service targets for efficient patient flow and hospital admission avoidance.
- Support the NETT team facilitating smooth transitions for complex neurological patients from hospital/rehab units back into the community. Your short term, intensive support (up to 12 weeks) will directly contribute to reducing hospital stays and preventing readmissions.
- Within CNRT, you'll work with individuals over 18 with a range of neurological conditions, providing vital support for recovery, managing difficulties, and promoting wellbeing. Your understanding of progressive conditions and the importance of ongoing neuro disability management will be key.
- Manage your case work with a degree of autonomy, utilising your professional judgment while benefiting from the support and guidance of your Deputy Team Manager.
- Empowering patients to achieve their goals and maximise their rehabilitation potential.
- Essential skills in delivering adult social care interventions, a strong understanding of the principles of neurorehabilitation, ability to work effectively in a fast paced environment with a focus on short-term interventions and achieving outcomes.
- Excellent communication and interpersonal skills to collaborate effectively within multidisciplinary teams and ability to manage your own workload and work with a degree of autonomy.

Essential Requirements:
Degree/Masters in Social Work and SWE registration.
2+ years of post-qualification experience.
Strong knowledge of UK social care legislation and practice.
