Crynodeb
Working with an integrated team including Health Visitors, Community Public Health Nurses (CPHNs), Admin, Early Years Outreach Workers (EYOWs), to deliver a virtual single point of access service for Health Visiting for the City of Birmingham. Utilising the Cisco/ARC/Jabber telephony system and Rio Clinical Record System, for which full training will be given To work in collaboration with the health visitor to deliver a holistic approach to supporting families, incorporating child development, public health and family support agendas. To work collaboratively with other relevant agencies where appropriate, establishing effective links to support signposting and referrals where required. To report back to the Health Visitor any concerns following any contact, in a timely manner. To assist in developing programmes of care to promote normal child development. To use a solution focused approach when working with families to support engagement with the service and maintain a child centred focus. To promote the importance of attachment and bonding, play and stimulation to encourage child development through interaction with parent and child. To provide advice, support and signposting around child development to include early interventions and onward referrals to AHPs to include SALT, ND Pathway & Childrens centre. Provide advice and support to parents regarding behaviour management individually or in groups, utilising presentation skills and working with colleagues from other agencies and within the locally agreed programmes. Provide health promotion information and advice to support work around the six public health high impact areas for the service, to include maternal mental health, breast feeding, healthy nutrition and weight, alongside other key public health agendas including immunisation, dental hygiene and smoking. Provide information and guidance in relation to accident prevention, home safety, child safety and safe play. Provide advice about treating common childhood illnesses and recognising when a child is ill. To provide guidance and support on childcare, nutrition and early education. To provide advice around parenting capacity and parental conflict and signpost to partners for additional support. To promote and support families to access the Childrens Centres and Family Hubs. To work flexibly as required, across the corporate team, to meet the needs of the service. To maintain clear and concise records. Skilled in reflective practice and able to evaluate own practice and share skills and learning. To enter all statistical information promptly onto computerised or manual records as required. To attend meetings and in-service training as required. To be aware of and respect the confidential nature of the work. To effectively respond to daily call and email workstreams supporting the requirements of families, partners and stakeholders. To undertake other duties commensurate with this grade of post in agreement with the relevant line manager. To minimise the Trusts environmental impact wherever possible, including recycling, switching off lights, computers, monitors and equipment when not in use. Helping to reduce paper waste by minimising printing/copying and reducing water usage, reporting faults and heating/cooling concerns promptly and minimising travel. Where the role includes the ordering and use of supplies or equipment the post holder will consider the environmental impact of purchases.
