An exciting opportunity has arisen for a clinical team leader to work within the home first service for the Salford locality. Home first supports the acute discharge team which ensures timely discharges from the acute site, the community hub which is the point of contact for all patients requiring support for discharge on pathways 1-3, the team complete all the mdt triage and ensure timely discharges can be undertaken- This clinical lead post will be the team leader for the hub but will also support the acute team and the rehab and reablement team.

You will be a proven leader who can motivate and support staff, have a clinical background either Nursing- Adult/ Mental Health, Social worker or Allied health professional.

As the Home First clinical lead you will assist and support the home first lead in the planning and organisation of the home first processes.
You will lead and be responsible for the performance management of services within the home first service.
You will act as the lead for the team motivating and supporting staff.

You will work as part of the Salford Adult Community Home First Team. providing clinical support, and advice to the MDT for patients who are approaching suitability for discharge or are medically appropriate for discharge and require assessment in their own home.

Facilitate a Multi-Disciplinary Team approach to triage of patients utilising the Strengths Based Approach to identify discharge pathways and provide clinical advice to identify those who may require support from another pathway e.g. Hospital at Home, Continuing Health Care, or may require step up to Urgent Care Team when already under the Home First Reablement and Rehabilitation service.

Act as the team leader for the service, providing supervision, training, and support to junior staff to keep patients well and engaged with rehabilitation at home and promote the Home First ethos.

Work closely with families and partner organisations, liaising and signposting to non-statutory services where identified and appropriate through Strengths-Based Conversations, keeping yourself and the team up to date with community-based initiatives.

This vacancy is open to those who have a current NMC PIN to practice as a Registered Nurse in the UK, or those who are currently enrolled on a course which means they expect to receive their full NMC Pin within the next 2-3 months.

Please note an NMC PRN Pin is not a full NMC Pin and would not fulfil the essential criteria for this role.
