More detail about the role An exciting opportunity has arisen for a Patient Flow Co-ordinator to join our team. The post holder is expected to provide efficient and effective flow coordination, working alongside our Medical, Nursing and Site Management teams to ensure that effective discharge planning and communication are maintained in order to provide a smooth, coordinated journey for the patient from admission to discharge or transfer to a base-ward. The post holders will be proficient in dealing with confidentiality issues and able to provide excellent customer care service, ensuring patients details are accurately recorded and the relevant documentation is created to support safe patient care. The successful candidate will demonstrate excellent communication skills, the ability to remain calm under pressure and experience of dealing with the general public and with difficult/distressed people. We are looking for someone who can offer great flexibility aside from their contracted hours, as the post holder will be required to undertake additional hours to cover for colleagues during periods of annual/sick leave. This post has been identified as involving access to vulnerable adults and in line with Trust policy successful applicants will be required to undertake an Enhanced Disclosure Check.
