Band 3 Health Care Assistant with the Learning Disability Team based at Erlegh House Reading and working in the community in Wokingham locality. This is a part time role, 30 hours per week, Monday to Friday, working pattern negotiable.



Interview will be held on 28th Nov 2024



You will be part of a multi-disciplinary team that is passionate about improving the health and lifestyles of people with learning disabilities.



Under the supervision of the Senior Community Nurse assist in the provision of a high quality individualised and person-centered service for clients with complex health needs in addition to their learning disabilities. The ideal candidate will be caring, empathetic and confident to work in a practical role working directly with clients.
• Promote the health and wellbeing of clients and encourage positive attitudes and practices.
• Provide support for clients who because of their complex health needs, cannot access or having difficulty accessing Primary and Hospital healthcare.
• To liaise and work jointly with the Community Nurses and other professionals in the team as required to meet the clients’ overall health needs.
