We at CAMHS Phoenix are lookingfora very enthusiastic,compassionateandproactiveindividual for the role ofSocialWorker, to join a verywell-establishedmultidisciplinary team.
Our service, based in CAMHS Phoenix on the Wokingham Community Hospital site, is a Tier 4, alternative to admission Day Hospital and Home treatment service, that provides evidence based psychiatric, nursing, and therapeutic interventions for young people between the age 12 – 18 years, presenting with severe and complex mental health difficulties assessed as requiring Tier 4 level care, including where these disorders are co-morbid with neurodevelopmental diagnoses.
All young people who are accepted to the service will follow a structured programme, in line with relevant NICE guidelines (for example anorexia nervosa, depression, anxiety, emotional dysregulation). Interventions are delivered by a multi-disciplinary clinical teamcomprisingof: Consultant Psychiatrist, Psychologists, Nurses, Occupational Therapist, Social Worker, Dietitian, Family Therapist, Teaching staff, Support staff including Assistant Psychologists, Clinical Support Staff, Activity Co-ordinator, Administrators and Managerial Support.
This will be a part time role - 22.5 hours per week
• To work with children, young people and their families who receive a service from the CAMHS Hospital at Home Team and who have been assessed as being children and families in complex need.
• To provide an expert social care perspective to CAMHS service development, as required.
• To provide experience andexpertiseto ensure the effective management of children at risk of abuse and neglect.
• Tofacilitatetimelycare and discharge planning from the service for children with complex multi-agency support needs.This involves a high degree of liaison and discussions with colleagues from multiple social care teams and other departments alongside the local authority (Benefits dept for example).
• To work as part of a multi-disciplinary team with specific responsibility for child protection, welfare issues and safeguarding, offering direct intervention tochildrenyoung people andfamilies. This will include provision of advice, consultation and guidance to colleagues working with complex cases to define and promote social perspectives of need, and to ensure the safeguarding of children and young people.
• To work closely with the CAMHS Phoenix youngpeople’local social care teams for the purposes of information-sharing, consultation, and access to mental health services, carrying out joint assessments and intervention whereappropriate tothe needs of the child/young person.

The “must haves” for this role:
• Registered with Social Work England
• Hold a full UK Driving Licence and have use of a car for work purposes
• Has experience of working within a mental health/multi-disciplinary setting andHasexperience of child protection work
