-To provide, create and deliver a range of peer support activities and care planning for service users who meet the Rough Sleepers initiative criteria -To provide specialist advice in relation to recovery to service users and actively encourage them to make positive changes to their lives -To be involved in developing the Co-production service and take a lead in recruiting volunteers to support service development. -To support the day to day running of the Rough sleepers outreach and assist service users to optimise support as well as fast tracking within the system . -To assist the running of Harm Reduction workshops within Hostels and at the Grove Drug service . Assist with Naloxone distribution among the Hostels and service user`s . -To assist needle exchange provisions within the community pharmacies and at the Grove. - To maintain clear and concise documentation as required - Seek to establish and maintain an effective working relationship with service users, relatives, carers, colleagues and other disciplines. -Ensure effective and confidential communication between professionals and service users. -Observe and report any concerns regarding service users to the nominated professional. - To ensure that any risk issues identified with service users are flagged up to the Manager. -To observe and report potential and actual hazards in any clients you work with to your Manager and senior colleagues and initiate common sense measures to alleviate these when necessary -To liaise with the manager and senior colleagues to ensure that Policies and Procedures are adhered to - Maintain confidentiality about any information concerning service users or colleagues that is of a confidential nature. -To participate in the Rough Sleepers and Clinical Team Meetings. - Ensuring that all Trust and team
