Please refer to the attached job description and person specification for full details of responsibilities To attend all educational sessions as directed by the University, submitting assignments promptly as required, to achieve all objectives to the standard required. To develop new skills and knowledge showing integration of theory and practice. To maintain a close liaison with the Practice Assessor throughout the course and to agree a clinical learning programme which will facilitate and support theoretical learning and reflect the value of practice centred education. Personal responsibility to identify and highlight any areas of concerns regarding progress and development (clinically, academically, and personally) before these become problematic. To increasingly understand the wider determinant of health and seek to address health inequalities across diverse communities and populations. Assess, plan, implement and evaluate individual/family focused programs of care (universal, targeted and specialist pathways), under the direction of the practice supervisor and assessor and work in partnership with other agencies e.g. support services for families, social care, early years provision/schools etc. Maintain professional record keeping standards ensuring records are accurate and contemporaneous in line with Trust policy and NMC standards. To develop an awareness and involvement in the delivery of the Healthy Child Programme with support and guidance from the Practice Supervisor and Practice Assessor whilst utilising the skills and knowledge of the wider health visiting / school nursing team. Developing into an autonomous practitioner throughout the course, taking responsibility for the care given to children / young people / families within own limits of competency and confidence. To be aware of and demonstrate increasing skills and knowledge in assessment and decision making in relation to safeguarding concerns associated to adverse childhood experiences (ACEs) Ensure concerns are responded to appropriately in line with local safeguarding policy and interagency safeguarding procedures and access appropriate training and supervision. Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs.
