ASW - Social Worker - Substance Misuse
Description
We are seeking a dedicated and compassionate ASW - Social Worker with expertise in substance misuse to join the employer's Social Care team. As an ASW - Social Worker - Substance Misuse, you will play a vital role in supporting individuals and families facing substance misuse issues, providing comprehensive assessments, and developing personalized care plans to promote recovery and overall well-being.

Responsibilities
Conduct thorough assessments to determine the level of substance misuse issues and their impact on the individual, their families, and the community.
Develop comprehensive care plans and interventions tailored to each individual's needs, incorporating evidence-based practices and therapeutic approaches.
Provide individual and group counseling sessions, offering support, guidance, and education on addiction recovery, relapse prevention, coping strategies, and life skills.
Collaborate with the treatment team to ensure coordinated and integrated care, including physicians, nurses, psychiatrists, psychologists, and other healthcare professionals.
Monitor progress and evaluate the effectiveness of treatment plans, making adjustments as necessary to optimize outcomes.
Advocate for patients' rights and access to appropriate services, resources, and treatments.
Collaborate with community organizations and social support networks to facilitate ongoing care and recovery beyond the hospital setting.
Requirements
Master's degree in Social Work or related field.
State licensure as an Associate Social Worker (ASW).
Experience working with individuals facing substance misuse issues and knowledge of evidence-based practices in addiction treatment.
Strong understanding of the impact of substance misuse on individuals, families, and communities.
Proficiency in conducting thorough assessments and developing individualized care plans.
Excellent communication and interpersonal skills, with the ability to establish rapport and build therapeutic relationships.
Ability to work both independently and collaboratively as part of a multidisciplinary team.
Strong organizational and time management skills, with the ability to prioritize multiple responsibilities effectively.
Familiarity with medical documentation and electronic health records systems.
Commitment to ongoing professional development and staying up-to-date with the latest research and best practices in substance misuse treatment.
