ASW - Team Manager - Independence & Access
Description
We are seeking a highly motivated and experienced ASW Team Manager to join our Hospital & Health Care organization. As the ASW - Team Manager - Independence & Access, you will play a crucial role in ensuring the smooth operations of our independence and access programs. You will be responsible for managing a team of ASWs (Associate Social Workers) and overseeing their daily activities.

You will collaborate with other healthcare professionals to develop and implement personalized care plans for patients, ensuring they have access to the resources and support they need to achieve independence. Responsibilities
Manage a team of ASWs and provide supervision, guidance, and support
Oversee the daily activities of the team, ensuring the timely completion of tasks and deliverables
Collaborate with healthcare professionals to develop and implement personalized care plans for patients
Assess patient needs and provide appropriate resources and support to promote independence
Conduct regular meetings with the team to review caseloads, identify challenges, and develop strategies for improvement
Monitor and evaluate the effectiveness of the independence and access programs, making necessary adjustments as needed
Provide ongoing training and professional development opportunities for the ASWs to enhance their skills and knowledge
Requirements
Bachelor's degree in social work or a related field
Current certification or license as a social worker
Minimum of 5 years of experience in social work, preferably in a hospital or health care setting
Strong understanding of the challenges faced by patients in their journey to independence
Knowledge of available resources and support systems for patients
Excellent leadership and communication skills
Ability to collaborate and coordinate with healthcare professionals
Strong organizational and time management skills
Ability to work independently and make sound decisions
Compassionate and empathetic attitude towards patients and their families
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:30
