We have an exciting opportunity for a Older Adults Recovery Coordinator who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs.
This role is a Fixed Term Contract that ends on 31st March 2025.
Where: Southampton
Hours: 37.5 hours per week
Salary: £27,449.53 - £31,529.41
About the role:
Ensure Older adults have access the Substance Use Disorder Service.
Monitor Older adults unplanned exits from the service and ensure robust support in place to enable older adults to stay in treatment.
Have a good understanding that older adults have distinct needs.
Understand that older adults may have co-occurring conditions of mental health and substance use.
Know that older adults may be on a range of different medications and have effective liaison with clinical team, GP, and Multi-disciplinary team meetings.
May need signposting for age-sensitive treatment such as support for sensory, cognitive, and physical health problems.
Might need interventions at a slower pace, involving repetition and provision of written information.
About you:
Considerable experience of working with people with Substance Use.
Experience of working with Co-occurring conditions.
Experience of working with Older Adults
Sound knowledge of drugs and alcohol and their effects
Knowledge of blood borne viruses and sexual health
Excellent communication skills
Case management experience
