Summary


This is an exciting opportunity for experienced Project Manager to work in Nottinghamshire county to support a new Health Innovation and Inequality fund (HIFF) project. The role will be to oversee a project that aims to improve the allocation of resources to transform and expand existing provision to create a new integrated SMD clinical team. Severe and Multiple Disadvantage (SMD) is a combination of three or more disadvantages. This HIIF funding will provide new clinical capacity in underserved areas with no or insufficient provision. This investment will transform existing fragmentated pieces of clinical provision for those with SMD into a dynamic, evidence based integrated offer that will address physical health, mental health and drug and alcohol dependency problems at the same time.

The project lead will be responsible for working with the strategy group to plan and deliver on this transformation of services, liaising with all providers and ensuring evaluation is measured and reported on. The project will be looking at how the physical health care clinicians, mental health clinicians, alcohol long term conditions team and drug and alcohol liaison teams can work more effectively together as a virtual team.



Manage the transformation (HIFF) project aimed at improving access to physical and mental health services for people experiencing SMD.

Work with the HIFF strategy group to identify the key drivers for the project and plan the work required.

Work autonomously, using own initiative, prioritizing workload, acting independently within appropriate guidelines, deciding when appropriate to refer to line management.

Identify areas of development and improvement in the county to enable services to work together, this may involve managing access to systems and information sharing agreements.

Offer advice and support to managers and across identified project area.

Requires management of complex, sensitive and contentious information

To work at sites across the county as necessary, we do not have a set base so the location in the county this can be negotiated with the successful candidate.

The postholder will be responsible for supporting, overseeing, implementing, and evaluating a variety of projects with the clear intention to improve access to services for people experiencing SMD – access to mental health, physical health and other community services as appropriate, within both Nottingham city and Nottinghamshire County services.
