THE PURPOSE OF THE ROLE General Practice is at the heart of the Health and Social Care system, and it is critical to have thriving, sustainable general practices that meet local peoples health needs. The PCN Clinical Pharmacist is instrumental in supporting their own practices clinically. This is a key role, intended to support the delivery and development of the pharmacist role in primary care and ensure that we continue to provide high quality, accessible and responsive services to both practices and patients. KEY RESPONSIBILITIES Acts within professional boundaries, supporting and working alongside a team of pharmacists and pharmacy technicians in general practice. Works as part of a multi-disciplinary team in a patient facing role. Takes responsibility for areas of chronic disease management within the practice and undertakes clinical medication reviews to proactively manage patients with complex polypharmacy. Provides primary support to general practice staff with regards to prescription and medication queries. (helps support the repeat prescriptions system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice) Provides clinical leadership on medicines optimisation and quality improvement and manages some aspects of the quality and outcomes framework and enhanced services. Ensures that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice. Demonstrates understanding of, and contributes to, the workplace vision. Demonstrates ability to improve quality within limitations of service. Areas of responsibility for all PCN pharmacists This role profile summarises the main responsibilities and requirements of the role only. The post holder will carry out any other duties as may reasonably be required by their line manager in line with the grade / band of the post. Job Description Agreement This job description is intended to provide an outline of the key tasks and responsibilities only. There may be other duties required of the post-holder commensurate with the position. This description will be open to regular review and may be amended to take into account development within the Practice. All members of staff should be prepared to take on additional duties or relinquish existing duties in order to maintain the efficient running of the Practice. Additional responsibilities To maintain the confidentiality of any information which comes into your possession regarding patients, employees, services or any other business relating to the organisation. To undertake any additional duties commensurate with the post and grade, as may be required from time to time, as directed by your line manager. Health and Safety To be aware of the responsibilities placed upon all employees under the Health and Safety at Work Act 1974, and to ensure the agreed safety procedures are practiced and understood Corporate Accountabilities - Standards of Business conduct The post-holder will be required to comply with the PCN's Standing Orders and Standing Financial Instructions, and always deal honestly with the PCN, with colleagues and all those who have dealings with the PCN, including patients, relatives and suppliers. This post is subject to a Criminal Records Bureau disclosure. The post holder is expected to adhere to the professional guidelines and codes of conduct of the General Pharmaceutical Council. Risk Management The post-holder will ensure compliance with the PCNs risk management policies and procedures and demonstrate a commitment to risk management, the recognition that our aim is to protect patients, staff and visitors from harm, and stress that all staff have a responsibility to minimise risk. Governance Standards Comply with the relevant governance standards applicable to the PCN and as communicated to the post-holder from time to time. Data Protection To comply with PCN Policies and the Data Protection Act in all respects, with particular relevance to the protection and use of patient information. Confidentiality The PCNs employees are required to ensure that information about patients is safeguarded to maintain confidentiality and is kept securely in accordance with NHS requirements. This means that a patient's information can only be passed to someone else with the patients consent or if it is essential to the effective provision of care for that individual. Child/Adult protection All staff must be familiar with and adhere to local and Statutory child protection procedures and guidelines, in conjunction with the Nottinghamshire Area Child/Adult Protection Committee policies and procedures. All staff are required to attend child/adult protection awareness training and additional training regarding child protection relevant to their position and role. Records Management Maintain PCN and patient records (both paper and electronic) in accordance with PCN and General Practice policies, in order to facilitate appropriate clinical care and effective administration. Security To comply with PCN policies to ensure there is a safe and secure environment that protects patients, staff, and visitors and their property, and the physical assets and the information of the organisation. Whistleblowing The post holder has a responsibility for patient and staff welfare and should raise any concerns relating to Professional Performance, Behaviour or Competence or a breach of NHS policies and procedures with their line Manager, other senior manager and/or external body. Performance review This Job Description will be used as a basis for individual performance review between the post holder and their Line manager. The Job Description covers only the key result areas and, as such, does not intend to provide a comprehensive list of objectives. Specific objectives will be reviewed annually and may need to be modified to reflect the changing needs of the role or the service.
