Crynodeb
1. Medicines Optimisation & Clinical Review Conduct structured medication reviews for patients with polypharmacy, frailty, multimorbidity, or high-risk medicines. Review and manage repeat prescriptions, ensuring safe and cost-effective prescribing. Provide clinical support for all longterm conditions, including hypertension, asthma, COPD, diabetes, cholesterol, Atrial fibrillation, etc. Within the scope of competence. Undertake medication reconciliation following hospital discharge, outpatient appointments, or care transitions. Identify and manage medicinesrelated risks, including monitoring requirements and safety alerts. 2. Prescribing & Deprescribing Prescribe and deprescribe medicines as an independent prescriber, working within clinical competence and practice protocols. Support GPs with complex prescribing queries and therapeutic decisionmaking. Implement evidencebased prescribing initiatives aligned with ICB and national guidance. 3. Patient-Facing Clinical Work Run clinics for medication reviews, longterm condition support, and medicines education. Provide advice to patients on the safe and effective use of medicines, including adherence and sideeffect management. Support selfcare and shared decisionmaking. To participate in all vaccination clinics for example flu, Covid etc To undertake home visits to care and nursing home including community when needed 4. Quality, Safety & Governance Lead on medicines audits, QOF indicators, IIF requirements, and prescribing quality improvement projects. Implement MHRA drug safety updates and ensure practice compliance. Contribute to antimicrobial stewardship programmes. 5. Practice Integration Work closely with GPs, nurses, HCAs, social prescribers, and other ARRS roles. Provide training and support to practice staff on medicines management processes. Participate in MDT meetings, case reviews, and clinical discussions. 6. Administrative & Operational Duties Manage medication queries from patients, community pharmacies, care/nursing homes and secondary care. Support repeat prescribing systems, synchronisation, and workflow optimisation. Maintain accurate, contemporaneous clinical records in line with professional standards. Complete workflow documents from Docman as per KPIs. To be responsible for all safety alerts. To run searches and perform audits as needed
