Community Palliative Care Nurse
Band 6

The community palliative care team are looking for an enthusiastic, motivatedregistered nurse to join Thundersley/Benfleet palliative care team as a band 6

The post holder will hold a caseload of end of Life patients and work as a key member of the Palliative Care Team delivering care alongside nurses and therapists to end of life patients. The post holder will work to reduce all inappropriate admissions to secondary care through the use of advanced care planning and practical high quality end of life care.

To provide holistic advice and support to palliative patients in the community. Working autonomously providing palliative care in the community. Symptom management is a key feature of the position. To liaise with other health care professionals and agencies to provide seamless care.
