Community Registered Nurse - District Nursing - Night Service

Are you self-motivated, enthusiastic and looking for a new challenge?
Would you like the opportunity to develop and further your nursing career in the community setting within the Night Service?
We deliver care from 9pm-8am seven days a week including weekends and bank holidays with a variety of shifts.
We are looking for a compassionate, hardworking and motivated registered nurse to join our Night Service Team in the South East Essex area.
You will have responsibility for Triaging calls/referrals and providing unplanned care to patients within the community setting. You will liaise closely with Out of Hours services and other members of the wider MDT as well as with patients and their families, therefore excellent communication and IT skills are essential.
There are many opportunities for further professional development.
You must be a first level registered nurse, hold a full valid UK driving licence and have access to a car.

• Referral triage
• Complex holistic assessments
• Hospital avoidance
• Palliative care
• Catheter care/bowel management
• Working collaboratively with the wider MDT
• Wound management
• Care planning
• Liaise with community nurse Team Leaders for future patient visits and allocation for additional support
• Excellent IT skills
