Location: Ipswich
Salary: £24,479 per annum pro-rata
Hours: 35 per week - Monday to Friday 10am to 6pm
Contract: Fixed Term Contract Until 30th September 2027
Driving: Full UK Driving Licence And Access To A Vehicle (Business Mileage Reimbursed)

We are looking for a passionate and enthusiastic person with great people skills to join our Health and Care service as a REACT Support Worker to provide reactive, focussed and flexible support to adults to live well and reduce vulnerability, which could be following a hospital stay to enable early discharge when clinical needs have been met or to those living in the community.

Our support workers are a friendly face to someone who may be struggling to do things for themselves due to age, illness, or vulnerability. You'll allow people to enjoy healthier, more fulfilling lives. A day in the life of a Support Worker will involve:
You will work within the REACT team which are part of a larger Multidisciplinary team compromising of medical staff, Occupational therapists, Mental Health, and other Health Care professionals.
Referrals are received from the clinical lead via community health professionals such as GPS and District nursing.
We provide support within the service users home to prevent a hospital admission by doing shopping, housework, prescription collection, respite sitting and welfare checks.
Some typical tasks might include:

Speaking with healthcare professionals about service user requirements
Driving to different locations including hospital, doctor surgeries, pharmacies, shops and patient’s homes
Offering both emotional and practical support to people using our service
Helping to assess our Service Users future needs and helping develop & create support plans and signpost to relevant agencies and community groups for support/engagement
Ensuring our systems are updated so that we can track what support has been offered to patients
To be a successful Support Worker, you’ll:
Have a full UK manual driving licence.
Have a good knowledge of services provided by the NHS and Social Care.
Be able to make things great. You know how to improve service quality for the benefit of users.
Be professional. You can deal with queries in a diplomatic and confidential manner.
Love being flexible. Working hours out with the norm suits you.
There is no personal care or administration of medication for this role.
