Work collaboratively with other health care professionals to provide shared patient care to promote best practice and allow patients to be cared for in their preferred place of care. Work with patients and families to create management plans addressing their individual physical, psychosocial and spiritual needs reflecting the holistic principles of palliative care and promoting dignity always. Work within the bounds of a multi-disciplinary team, contribute to multi-disciplinary discussions and refer to other members of the team to enhance patient care. To demonstrate effective communication skills with patients, carers and the multi disciplinary team both face to face and via the telephone to share and gather information, offer support and advice and initiate conversations enabling informed decisions about future care planning. To accurately document all aspects of patient care and significant conversations. Provide supervision and undertake Appraisals with Health Care Assistants to support them in areas in which they need development. Recognise signs of dying to enable discussion with patients and families regarding choices of care, to initiate anticipatory prescribing so that symptoms can be addressed and inform other health care professionals. To ensure the appropriate ordering, storage and recording of medications and other clinical supplies. To maintain professional relationships promoting trust and integrity. To take part in clinical audit and undertake risk assessments in line with Southern Hospice Group clinical governance. To delegate work to junior colleagues as required for best possible quality and continuity of patient care. To demonstrate skills of advocacy by promoting the patients best interest. To shift lead in the absence of more senior staff. Develop knowledge in symptom control and palliative care emergencies. Work within the bounds of a multi-disciplinary team, contribute to multi-disciplinary discussions and refer to other members of the team to enhance patient care. Participate in internal and external meetings providing a level of expertise that will enhance Southern Hospice Group credibility. To be involved in setting and achieving team and service objectives. To respect patient/family confidentiality Education and Personal Development. To attend mandatory and statutory training sessions and competencies and any other relevant study opportunities to ensure continuing up to date knowledge. To maintain own professional NMC revalidation requirements and plan on-going learning and development with appraiser. To receive clinical supervision and be aware of the emotional demands of the post and maintain personal wellbeing. To provide support and act as a clinical resource for colleagues in their educational development. To demonstrate regular reflective practice. To act as a mentor for junior colleagues
