The main responsibilities of this role include: To work autonomously, with access to the DiabetesSpecialist Team, in providing evidence based and highquality diabetes care and education To assess, plan, implement & evaluate individualisedprogrammes of evidence based care providing specialistDiabetes advice to patients, carers/relatives, and themultidisciplinary team. To contribute to the organisation and delivery of ongoingeducation and training for other healthcare professionals.In conjunction with the Diabetes Multidisciplinary teamassist in the development, implementation and adoption oflocal diabetes care policies and guidelines.
