Crynodeb
To provide face-to-face consultations, telephone consultations and home visits for housebound patients, dealing with queries, paperwork and correspondence in a timely fashion To make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation To undertake and request pathology, radiology and general screening tests as clinically appropriate and following up on all results and take appropriate action within an agreed upon time-frame Assessing the health care needs of patients with undifferentiated and undiagnosed problems Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate. To aid in the signing of acute and repeat prescriptions as presented on a previously agreed schedule. Avoiding the use of hand-written prescriptions, where possible. To use the clinical system (TPP SystmOne) to record clear and contemporaneous computer based consultation notes to agreed standards Referring patients externally, where appropriate, to local teams and organisations under local guidelines or utilising the in-house clinical team for further opinion as needed. Working closely with the administrative team to manage caseloads, arrange follow-up, initiate referrals and deal with any day-to-day queries which may arise. Providing counselling and health education Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate To attend regular on-site clinical meetings with the Partners and other members of the clinical team as requested To provide a full range of medical services, as defined in the core GMS contract and additional enhanced services where appropriate To undertake all the normal duties and responsibilities associated with a GP working within primary care.
